Who We Are?
ABOUT US
To ensure stability of care, the patient will keep his or her current primary care physician. The patient’s physician will collaborate with you, your family and the hospice to ensure the best quality of service. The hospice team is available 24 hours a day, every day. The hospice team consists of experts in their individual fields:

Hospice Medical Director
Registered Nurses
Social Workers
Certified Home Health Aides
Trained Volunteers
Chaplain
Therapists (Speech, Physical, Occupational, and/or Respiratory)
